DirectoryIf you're considering or starting feminizing hormone therapy, here's what changes to expect and when. Every body is different — this is a general guide.
Breast buds begin (sore, tender lumps). Skin starts softening. Decreased sex drive and spontaneous erections. Some scalp hair regrowth may begin. Emotional changes start — you may feel a wider range of emotions. Body scent begins changing.
Faster breast growth. Fat begins redistributing (face, hips, thighs). Skin is noticeably softer and clearer. Muscle mass decreasing. Body and facial hair thinning. Testicle size decreasing. Acne often clears up.
Fat redistribution becomes more visible — facial shape, hips, and thighs changing. Breast growth continues. Body hair patterns shifting toward female distribution. Eye shape may change. Height and shoe size may decrease slightly. Orgasms may feel different — many report more full-body experiences.
Breast development approaches maximum. Fat redistribution continues refining your shape. Body hair continues thinning. Facial feminization from fat redistribution can be significant. Skin transparency increases. Most changes are well underway — this is where patience pays off.
Permanent changes: Breast growth, hip bone growth (if under 25).
Reversible if HRT is stopped: Skin softness, fat distribution, muscle changes, body hair, sex drive changes.
Not affected by HRT: Voice (requires voice training), facial hair (requires laser/electrolysis), bone structure from male puberty.
Fertility: HRT will likely affect fertility. Consider sperm banking before starting.
Wait 2–3 years before considering facial or breast surgery to allow full development.
